Empowering Hillingdon residents to take control of their health
My Health Hillingdon’s aim is to engage, educate and empower those living with long term conditions and other health issues, both physically and mentally to enable them to better self-care, self-manage and navigate services by increasing knowledge, skills, and confidence. They do this by providing high quality FREE workshops all facilitated by a health professional, giving you the chance to learn and ask questions you would not get time for in a normal appointment
The philosophy of the programme is to ensure that the residents of Hillingdon can access high quality, evidence-based care in a setting appropriate to their needs by transforming the way care is delivered by co-producing the programme with service users and carers, providers, the local authority and community and voluntary sector groups.
