Job Description: Registered Nurse with an interest in Palliative Care and End of Life Care, responsible for delivering a high standard of care to everyone staying at our 13 bed Inpatient Unit.

Location: Inpatient Unit at Michael Sobell House, Mount Vernon Hospital

Working Hours: Nights plus bank hours

Salary: NHS Band 5 equivalent (Existing NHS pensions honoured or workplace pension available)

Job Description: Harlington Care provides support 24 hours a day, 365 days a year. Work ranges from domiciliary care to respite support in an individual’s own home and group activities in the community.

You will coordinate care and support for individuals who frequently use urgent and emergency care services, working in partnership with health, social care and voluntary sector professionals to improve outcomes and reduce health inequalities.

You will identify and monitor individuals using a service dashboard, coordinate multi-disciplinary team meetings, develop tailored care plans and ensure referrals and actions are followed through appropriately.

Location: London Borough of Hillingdon.

Working hours: Full time, 35 hours per week.

Flexible to meet client needs.

Salary: £30,000 per annum + contributory pension.

You will manage a caseload of High Intensity clients who frequently use urgent and emergency care services, providing person-centred floating support to improve wellbeing and reduce non-elective admissions, A&E attendance and unscheduled care interactions.

You will work directly with clients with complex needs, including mental health, addiction, homelessness and social challenges, supporting access to appropriate services, advocating at appointments and attending multi-disciplinary team meetings to ensure coordinated care.

Location: Harlington Hospice, Mount Vernon Hospital, Gate 3, Northwood HA6 2RN, with community-based work across the London Borough of Hillingdon.

Working hours: 35 hours per week.

Flexible working to meet the needs of clients.

Salary: £29,000 per annum + contributory pension.

Reports to: HIU Service Manager.
